Health Statement
This form can be completed by the horse owner or agent stating the overall health and soundness of the horse. Used for horses that do not require a veterinary examination. Submit along with the Owner’s Application.

Veterinary Examination
This form must be completed by the veterinarian for horses that have had a medical issue in the previous year, under age/over age horses, or higher value horses. Please contact us for exact guidelines. Submit along with the Owner’s Application.


Loss of Use Examination
This form in addition to the Veterinary Examination form must be completed by the veterinarian for combined internal/external loss of use coverage.  Submit along with the Owner's Application.  Submit the Veterinary Examination form if only external loss of use coverage is needed.


Farm Owners' Application
Application for property and liability coverage for your horse farm. Includes coverage for damage to your residence, farm buildings, personal and farm property, as well as liability coverage for property damage or bodily injury to others as a result of your personal or commercial farm operations.


Commercial Equine Liability Application
Application for liability coverage for property damage or bodily injury to others as a result of your commercial equine operations. Horse-related businesses, including boarding stables, breeding farms, trainers and sales barns should consider this coverage.


Care Custody and Control Application
Application for coverage if you are found to be legally liable for injury or death to a non-owned horse in your care. Boarding stables, breeding farms, trainers and other commercial equine operations who take on the responsibility of caring for others’ horses should consider this coverage.


Private Horse Owners' Liability Application
Application for liability coverage for property damage or bodily injury to others as a result of your personal horse ownership. This application is for the individual who is not a professional horseman or involved in a commercial equine operation.


Riding Club Application
Application for liability coverage for property damage or bodily injury to others that occurs during club activities. Clubs and organizations that hold mounted events which are open to the public should consider this coverage.

Horse Show / Event Application
Application for annual or short-term events and shows. Provides protection for property damage or bodily injury to others as a result of accidents during an equine related function.
